HOW TO REGISTER YOUR CHILD

Please click the button below to send an email asking to be registered in the dance classes you are interested in joining.

Enrollment Form

 

To register your child into any of our classes please could you complete the enrollment form in full. This form provides us with your contact information, medical information and consent for photographs and first aid treatment should it be required - so it is a very important form.

​

The information on this form is stored in accordance to GDPR using in the cloud storage via Google Drive. Only staff members from Oak Tree Dance Studio will have access to this information.
